Pay Range: $62,337.18 - $77,634.04
Under the general supervision of the Director of Health Home Integrated Care, provides leadership and coordination in the clinical practice of the Specialty Health Home delivery system within the Saginaw County Community Mental Health Authority (SCCMHA) provider network. Provides leadership in the methods of clinical service delivery that support health home concepts and integrated care, in particular strengthening care coordination relationships between Behavioral Health and Primary Care. Functions as a specialist consultant to the primary treatment teams throughout the SCCMHA network and the scope of work relationships are internal to the network. Works closely with the Health Home Primary Care Coordinator through the focus of communications internal vs. external relationships. Facilitates administratively the patient specific care coordination process. This position will be knowledgeable about and actively support culturally competent recovery based practices; person centered planning as a shared decision making process with the individual, who defines his/her life goals and is assisted in developing a unique path toward those goals; and a trauma informed culture of safety to aid consumer in the recovery process.
- Bachelor s degree in Nursing.
- A Master s degree in Nursing or Healthcare Administration is preferred.
- Valid Michigan Driver s license with a good driving record.
- A licensed registered nurse (RN).
- Five (5) years of experience in nurse education and team leadership. Experience may be through formal job duties or informal work assignments in which team leadership resulted in implementation of process improvements. Previous supervisory experience in healthcare or related organization preferred.
Knowledge, Skills and Abilities:
Knowledge and ability to apply the methods, practices, protocols which are Health Home evidence based practice; the Triple Aim model of health quality improvement; the electronic medical record technology related to decision support and quality management concepts and methods.
Understands the health business environment.
Ability to implement methods and principles of adult education and identify ethical and cultural aspects of situations when making decisions.
Strong leadership skills are required and the ability to function as a member of a team is critical.
Effective oral and written communication skills, minimum at college level. Communicates ideas and information verbally and in writing and is able to prepare documents and presentations in multiple media. Is able to tailor communication to the reader in format and complexity.
Computer competency, including word processing, spreadsheets, databases, internet, E-mail.
Ability to exercise independent and mature judgment.
