Job DescriptionSocial Worker (LMSW)*PRN - Home Health Care**Competitive Visit Rates, Paid Mileage, and Flexible Schedule!**As a Social Worker, you’ll provide and facilitate the acquisition of economic, social, and emotional support services to our patients and their families and/or caregivers to treat geriatric patients in home, community and senior living facilities with regard to Agency policy and accepted professional standards of practice.QUALIFICATIONS: Master’s Degree from a school of Social Work accredited by the Council on Social Work Education. Fully licensed MSW for the State of Michigan (LMSW) Minimum of one (1) year of experience in a health care setting, home care experience preferred. Willingness to travel throughout the Midland area. Strong communication and interpersonal skills. Current CPR certification. Must be able to pass a comprehensive background check and drug screening.ESSENTIAL DUTIES AND RESPONSIBILITIES: Assesses the psycho-social status of clients to determine factors that may interfere with client’s ability to achieve goals. Develops a written plan to provide information and direction to other health team members. Documents all findings, plans, interventions, and client progress in the clinical record. Communicates plans and changes to the physician and/or nursing supervisor and other Agency staff through the care plan, written progress notes, and participation in care conferences. Initiates referrals as appropriate. Educates team members and clients/families/caregivers about community resources and how to access them appropriately. Prepares social histories to augment existing service, or as a guide in determining or changing level of service.
