Job DescriptionJob Opportunity: Offers a full time position to a qualified Physician Assistant to provide primary care medicine services to the homebound.Candidates must have at least 2 years clinical experience in any of the following areas: family practice, internal medicine, rehab medicine, neurology, cardiology or geriatrics.Candidate must be able to practice independently with strong clinical decision making; be able to communicate clearly with patients and caregivers and be able to navigate well through the EMR.The best candidates will also possess a respectful, kind and mature demeanor.Job DutiesPhysician Assistant or Nurse Practitioner shall:· Provide primary medical care for patients in their home.· Perform a complete history and physical on new patients. May exclude genitalia exam.· Assess the status of chronic conditions and acute issues at each visit.· Complete a review of systems at each visit.· Create a problem list or diagnosis list (assessment) and update at each visit if needed.· Formulate a treatment plan based the H&P and Assessment / Problem List.· Prescribe appropriate medications and evaluate their effects in terms of efficacy & safety· Review and reconcile medications at each visit.· Order and interpret indicated test results.· Manage patient disease states.· Implement health maintenance & preventative health measures.· Educate caregivers and patients at each visit.· Communicate with patients, caregivers and other healthcare providers in compliance with all applicable privacy laws and policies.· Meet with the patients family to discuss medical care and end of life issues as indicated.· Communicate with physician as outlined in the practice agreement.· Participate in case conferences with physicians and mid-level providers as needed.· Document all information noted above in Practice Fusion in a clear and concise manner.· Supervise medical staff team including medical assistants and homecare nurses and therapists.· Assist in the oversight of homecare nursing and therapy.· Practice medicine within the scope of licensure/certification/ and education within a housecall setting and according to a practice agreement.· Take after hours phone calls one week a month.· Demonstrate competence using the Electronic Medical Record (EMR) system Practice Fusion· Demonstrate competency using the electronic fax machine (Updox) including reviewing, completing, signing and forwarding patient care documents.Responsibilities typically delegated to a medical assistant:o Give injections of appropriate medicationso Check vital signso Update medication list and assist in completing medication refillso Schedule follow-up appointments.o Call the patient one business day prior to next appointment.o Schedule referrals/consults for specialists.o Track lab and test resultso Send orders for labs and special tests.o Complete patient instruction sheets ( sheets) or fax visit note plan of care to patient or caregiver.Perform minor procedures (per skill level)o Peripheral joint injectionso I&D abscesses or boilso Joint aspirationo Mole removalo Skin tag removalo Wound debridementAdministrative duties:o Check email each business day.o Check EMR messages each business day.o Complete visit notes in a timely manner: 2 business days for a new patient visit and 1 business day for follow up visits preferably at the time of the visit.o Oversee homehealth nursing and therapy as indicatedo Submit careplan oversight minutes to support staff on a regular basis but no less than once a calendar month.o Maintain a thorough and complete patient chart.o Participate in CMS Quality programs as directed by practice manager or CEOWork Standard:· A full work day is considered 8-12 patients per 8 hours. Full time work 40-50 visits per week.· Work days are Monday through Friday typically 8:00 am-5:00 pm Saturdays are optional; Saturday visits should be approved by patient or caregiver. We request all staff to express these values during their work.Physician Assistant Compensation and Benefits Full time: 5 days per week with an average of 9 - 12 patients per day. Base pay (guaranteed): $80,000 per annum Per visit rate (Paid bi-weekly in lieu of base pay when total bi-weekly per visit rate exceeds base pay rate): $50 for a new patient visit (billing codes 99327,99326, 99344, 99343) $43 for a chronic disease management visit. (billing codes 99349, 99336) $30 for an acute illness visit (billing codes 99348, 99335) Productivity : must maintain an average of 36 visits per week to qualify for per visit rate. PTO: 160 hours per annum at base rate CME reimbursement: $1000 per annum Professional liability insurance premium: covered 100% Major Medical Health care insurance: 100% premium covered for employee and 50% premium coverage for dependents. Health Savings Account available $250 per day when on-call for a Holiday - Christmas, Thanksgiving, Fourth of July, Labor Day, Memorial Day, New Years Day.
