Job DescriptionThe Case Management Specialist demonstrates excellent customer service performance in that his/her attitude and actions are at all times consistent with the standards contained in the Vision, Mission and Values of Covenant and the commitment to Keeping our Promise of Caring. This individual is responsible and available on an as needed basis for the case management of a group of patients within specific disease entities. This includes facilitating the coordination of multiple disciplines and resources to provide optimal care to the specified patient population. This individual promotes collaboration with the patient care team to ensure a high quality of patient care, reduced length of stay, efficient utilization of resources, and appropriate reimbursement of care and services. The Case Management Specialist is responsible for a specific patient population and will follow those patients with complex needs throughout the continuum of health care, including development of the transition plan. This individual utilizes the EHR process appropriately. This individual supports the Clinical Documentation Management Program (CDMP) by ensuring documentation accurately reflects acuity and treatment given, serving as a clinical resource for medical record coders, and coordinating communications with physicians. The Case Management Specialist will refer information for third party payer contact to the CRM Resource Center the Concurrent Care Manager decision support system.Responsibilities:Contributes to organization success targets for patient satisfaction by meeting the Case Management Specialist Expectations for Customer SatisfactionContributes to organization success targets for net operating marginEnsures the availability of accurate and timely informationCollaborates in the development, revision and evaluation of a multidisciplinary plan of care by continuously assessing patient and family needs from pre-hospitalization through post hospital servicesInitiates and manages transitions when the patient experiences the need for new and different services; collaborates with and uses support of Social Worker regarding cases with complex social service needsInitiates and manages transitions when the patient experiences the need for new and different services; collaborates with and uses support of Social Worker regarding cases with complex social service needsWhen possible, prior to patients admission to the hospital, meets with him/her to provide education, identify post transition needs and set patients expectations regarding course of recoveryEducation:RN with current license in State of MichiganBachelors degree in a health care field preferredSkills:Demonstrates excellent customer serviceDemonstrates critical thinking skills, analyzing multiple issues impacting outcomesExcellent problem solving skills and the ability to manage many situation simultaneously; able to adjust to priorities that may change minute by minuteDemonstrates good computer skillsDemonstrates excellent communication skills, negotiation skills, diplomacy and assertivenessAble to sit for extended periods of timeAble to be on feet and walk for extended periods of timeAble to lift, bend, and carryCredentials:RN with current license in State of MichiganCase Management Certification preferredExperience:RN with 10 years recent acute care clinical experience or 5 years recent case management experience preferred or RN with 3 years recent acute care setting experience and holds a bachelors degree requiredCharge responsibility experience strongly preferredDemonstrated clinical competenceHas exceptional understanding of the disease process and treatment regimens associated with designated patient populationsMaintains current knowledge by attending conferences, seminars and reads journal or research articlesStrong commitment to collaboration and teamwork, with demonstrated ability to work as a member of a team where assignments must be coordinated with peersHas a solid understanding of the Healthcare industry, technology and regulationsA professional approach to work, including a strong sense of responsibility for assigned dutiesCompany DescriptionCovenant HealthCare is a regional health care leader that offers a dynamic and supportive working atmosphere.
