Job DescriptionThe Case Management Specialist demonstrates excellent customer service performance in that his/her attitude and actions are at all times consistent with the standards contained in the Vision, Mission and Values of Covenant and the commitment to Keeping our Promise of Caring. This individual is responsible and available on an as needed basis for the case management of a group of patients within specific disease entities. This includes facilitating the coordination of multiple disciplines and resources to provide optimal care to the specified patient population. This individual promotes collaboration with the patient care team to ensure a high quality of patient care, reduced length of stay, efficient utilization of resources, and appropriate reimbursement of care and services. The Case Management Specialist is responsible for a specific patient population and will follow those patients with complex needs throughout the continuum of health care, including development of the transition plan. This individual utilizes the EHR process appropriately. This individual supports the Clinical Documentation Management Program (CDMP) by ensuring documentation accurately reflects acuity and treatment given, serving as a clinical resource for medical record coders, and coordinating communications with physicians.
